Steve Arendt raised nine bullet points within Keep a Sense of Vulnerability. The first three echo what I personally heard from Trevor Kletz when we taught a short course for the AIChE in the mid-1980s.

Hopefully you are very familiar with the superb process safety awareness provided by the monthly publication of the Beacon. This tool was fiilly discussed in the earlier chapter, which discussed the need for imease, or keeping a sense of vulnerability, but it will be paraphrased here. The Beacon is a 1-page image rich lesson sponsored by the American Institute of Chemical Engineers (AIChE). It is available free of charge and loaded with contributions from a team of international volunteer experts. 

Chapter 15 is entitled Keep a Sense of Vulnerability for Safety Safe. 1 appreciate the input from two masters. 1 thank Joseph Louvar for encouraging me to write the topic as an AlChE Process Safety Progress article and to help shape it 1 am grateful for Mark Rosenzweig, Editor-in-Chief, of Putman s Chemical Processing magazine for his skillful enhancemerrts to polish the chapter. 

Such examples show why technology is never a panacea, in the sense that whenever one introduces a technology to reduce one kind of error, one is likely to introduce the possibility of new kinds of error. Extensive road testing in real settings is the only way to reveal these vulnerabilities and points again to the need to keep the realities of the clinical world constantly in mind during development.
