Incident right

I didn t tell my mother about the incident right away because I didn t want her to worry. But I decided that from then on I would camp in more populated areas. The problem with that, though, was that I had to move several times a day and several times a night because I d get wind from a camp fire or somebody using bug spray. 

What if I don t learn about an incident right away If you do not learn of a report-able incident at the time it occurs and the incident would otherwise be reportable under paragraphs (a) and (b) of this section, you must make the report within eight (8) hours of the time the incident is reported to you or to any of your agent(s) or employee(s). 

The last relation in equation (Al.6.107) follows from the Fourier convolution theorem and tlie property of the Fourier transfonn of a derivative we have also assumed that E(a) = (-w). The absorption spectmm is defined as the total energy absorbed at frequency to, nonnalized by the energy of the incident field at that frequency. For a circularly polarized light experiment, one can measure the cross sections for either right (r) or left (1) polarized scattered light. Suppose that right polarized light is made incident on a Raman active sample. The general expressions for the Raman cross sections are [176]... 

Equation (B 1,9.11) is valid only for plane polarized light. For unpolarized incident light, the beam can be resolved into two polarized components at right angles to each other. The scattered intensity can thus be expressed as (figure Bl.9.2)... 

If, instead of assuming diffuse reflection at the wall, it is postulated that a fraction f of the incident molecules is scattered diffusely and the rest suffer specular reflection, the right hand side of equation (2.8) must be multiplied by a factor (2 - f)/f. ... 

The second term on the right-hand side, a component oscillating at frequency 2co, represents the second harmonic of the incident beam. This component of the polarization vector can radiate light at the frequency 2co. Observation of the second harmonic generation was demonstrated in the early 1960s using mby lasers (59). 

The client may want to reserve the right to participate in investigations of incidents or releases resulting from production, packaging.

Injection—The injection of hazardous materials into the body sounds, at first, like a bad joke. Who in his right mind would inject themselves—exp>ecially when it is not required by a doctor However, it can occur by stepping on or bumping against a sheup object while working at an incident site. It will happen before you even realize it has occurred and the reality of possible being internally contaminated sinks in. The best precaution for this eventuality is to have on protective clothing and observe safe work habits.

When an electrical supply has been isolated, it is normal practice to check that the right switches have been locked or fuses removed by trying to start the equipment that has been isolated. However, this system is not foolproof, as shown by the following incidents. 

When replacing relief valves, care must be taken to make sure that the right relief valve is replaced. Section 6.1 (d) describes another incident that occurred when a flare line was broken. Valves of different internal sizes may look alike (see Section 1.2.4). 

Note that, as in the incident described in Section 13.3, the operation had been carried out a number of times before conditions were right for an explosion to occur. 

Read it right through. As you do so, ask yourself if the incidents could occur in your plant, and, if so, write down what you intend to do to prevent them from occurring.
