Idiopathic form

Benign familial neonatal convulsion is an idiopathic form of epilepsy beginning within the first six months after birth. Seizures include generalized and mixed, starting with tonic posture, ocular symptoms, and apnea, and often progress to clonic movements and motor automatisms. 

Deficient parathyroid hormone secretion most commonly results from neck surgery, and idiopathic forms are rare. The symptoms and signs are those of hypocalcaemia, with paraesthesiae of the hands and peri-oral area, non-specific muscle weakness, gastrointestinal upset and tiredness. 

The demonstration that iron deficiency was an important risk factor in RLS pathogenesis has led, since the 1950s, to several therapeutic attempts with iron supplements. Initially, these attempts were aimed at subjects with RLS and hyposideremia, with or without anemia later partial supplementation was also tested in patients with the idiopathic form.

The differential diagnosis includes idiopathic torsion dystonia, parkinsonism, idiopathic torticollis, Huntington s disease, Wilson s disease, and Meige s syndrome (blepharospasm, oromandibular dystonia). A retrospective evaluation of the records of patients with idiopathic cervical dystonias (n = 82) and tardive cervical dystonias (n = 20) has been performed, in a search for clinical features that could help separate these closely related disorders (332). Despite the overall similarity, the presence of a dystonic head tremor was strongly suggestive of the idiopathic form, which was present in 42% and did not occur at all in the tardive group. A family history of dystonia (10%) was also exclusive to the idiopathic group. 

Environmental Toxins. Even in disorders with strong familial association, a role of shared environmental exposures and other indirectly related factors must be considered. Studies of familial but idiopathic forms of PD-like neurodegenerative disorders in the Western Pacific do not support genetic or infectious etiologies, but leave open the possibility of toxic factors (46, 47). 

Neurodegenerative processes can involve diverse areas of the CNS. The initial etiology of such disturbances is mostly unknown. Nevertheless, we can distinguish between an idiopathic and a symptomatic form of neurodegenerative disease. A close examination of the latter enables one to make valuable conclusions about the nature of the former. Thus, we can amplify our knowledge by examining function, structure and biochemistry of the CNS in symptomatic cases and by looking for comparable alterations in idiopathic forms. 

Clinically, the patients present with a rod-like painless induration in the dorsal aspect of the shaft. Clinical signs and symptoms suggestive for throm-bophebitis, such as fever and local pain, are not associated with the idiopathic form.
