At the heart of patient safety in healthcare is the avoidance of medical error, the failure of a planned action to be completed as intended, or use of the wrong plan to achieve an aim [5], Patient safety involves the study of medical error not for legal or punitive purposes but rather to help us understand the factors which contribute to adverse incidents, to enable learning and prevent harm in the future.
