PHARMACOLOGICAL EEEECTS AND MECHANISMS OF ACTION Topiramate reduces voltage-gated Na+ currents in cerebellar granule cells and may act in a manner similar to phenytoin. In addition, topiramate activates a hyperpolarizing K+ current, enhances postsynaptic GABA -receptor currents, and also limits activation of the AMPA-kainate-subtype(s) of glutamate receptor. Topiramate also is a weak carbonic anhydrase inhibitor. [Pg.332]
