Final Report and Document

After an entry has occurred and the person who entered the confined space has completed his or her work, a final report should be written. This report must include  

Reason(s) that this confined space had to be entered. 

A list of alternative methods explored and reasons that these alternative methods were determined unfavorable.  

Any issue(s) that may come up during the preparation stage.
