Example FMEA Worksheet

The assessor can start compiling the FMEA worksheets once the scope of the FMEA is agreed. Tables 5.1-5.7 provide some example FMEA worksheets. 

The analytical technique used for this study was the failure mode and effects analysis (Figure 14-3 shows the FMEA worksheets filled out for the above Sprinkler example). 

The completed FMEA worksheet (Figure 10.4) is provided to demonstrate its use in evaluating the following examples of identified failme modes.
