The role of systemic corticosteroids for acute exacerbations of COPD has been clarified in recent years. However, the appropriate dosage regimen is not well established. Regimens range from initial high doses (methylprednisolone 125 mg every 6 hours) to more conservative dosing (prednisone 40-60 mg/day).

Corticosteroids play a key role in the management of SVCS, particularly in cases of lymphoma, because these tumors inherently respond to corticosteroid therapy. They are also helpful in the setting of respiratory compromise. Corticosteroids benefit patients who are receiving radiation therapy by reducing local radiation-induced inflammation and increased intracranial pressure. Dexamethasone 4 mg intravenously or by mouth every 6 hours is a frequently used regimen. The dosage should be tapered on completion of radiation therapy or resolution of symptoms. 

Aprepitant is supplied in 80- and 125-mg capsules and is administered for 3 days in conjunction with highly emetogenic chemotherapy along with a 5-HT3-receptor antagonist and a corticosteroid. The recommended adult dosage of aprepitant is 125 mg administered 1 hour before chemotherapy on day one, followed by 80 mg once daily in the morning on days two and three of the treatment regimen. 

A 60-year-old woman was referred for management of severe rheumatoid arthritis. She had had the disease for 15 years and had been treated until age 55 with aspirin. She then switched to ibuprofen, which diminished the gastrointestinal adverse ects she had developed from aspirin. One year before referral, she started to complain of increased joint pain and stiffness, and laboratory studies confirmed that the disease had become more active. Several attempts to control her symptoms with increased dosage of ibuprofen and with a trial of another NSAID were not successful, and the decision itm made to add corticosteroids to the regimen.
