Disaster Medical Assistance Teams. NDMS helps to develop local DMATs. Each DMAT is sponsored by a major medical center and comprises approximately 35 physicians, nurses, technicians, and administrative support staff designed to provide medical care during a disaster.

The formation of ergot alkaloids from L-tryptophan is well-known (Herbert, 1989 Groeger and Floss, 1998 Markert et al., 2008). L-Tryptophan condenses with dimethylallylpyrophosphate (DMAPP) to give y,y-dimethylallyltryptophan (DMAT), which is modified via chanoclavine 1 to give agroclavine and finally elymoclavine (Fig. 2.14). Lysergic acid maybe formed from this last alkaloid. 

The enzyme responsible for the first step in the biosynthesis of these alkaloids is DMAT synthase. This enzyme, which brings about the condensation of L-tryptophan with DMAPP, has been isolated from C. purpurea cultures. 

We have therefore selectively blocked the first enzyme in the biosynthesis pathway of EA (DMAT-synthase) by 5-fluorotryptophan (Scheme 8). This treatment id not influence other biochemical processes in the living cells of C. purpurea, thus enabling the fructosylation of introduced alkaloids to be performed [27].

The biosynthetic pathway to the ergoline nucleus proceeds through 4-dimethylallyl tryptophan (4-DMAT), chanoclavine-I, agroclavine, and lysergic acid. Two cis, trans isomerizations occur one before chanocla-vine-I and the other before agroclavine, as shown by experiments with [2- C]-mevalonic acid and [Z-CH3]-4-DMAT (Fig. 36). The peptide unit is derived from a combination of three amino acids, one of which is always proline. Several genera in the plant family Convolvulaceae Rivea, Ipomoea, etc.) also produce ergot alkaloids.

DMAT members must maintain the appropriate certification and hcensure within their specific professional disciphne. When activated, DMAT members become Federal employees, with all States recognizing their licensure and certification. In addition, DMAT members receive pay while serving as part time federal employees and they receive liability protection through the Federal Tort Claims Act (20). Although DMATs serve primaiily as a local community resource available to support local, regional and state needs, as a national resource they are subject to federalization and deployment elsewhere (20).
