Disorders of coagulation

Genetic disorders of coagulation factors occur, and the two most common involve factors VIII (hemophilia A) and IX (hemophilia B).

Single low doses of aspirin (81 mg daily) produce a slightly prolonged bleeding time, which doubles if administration is continued for a week (see Chapter 34 Drugs Used in Disorders of Coagulation). The change is due to irreversible inhibition of platelet COX, so that aspirin s antiplatelet effect lasts 8-10 days (the life of the platelet). 

Desmopressin is also used for the treatment of coagulopathy in hemophilia A and von Willebrand s disease (see Chapter 34 Drugs Used in Disorders of Coagulation). 

Several disorders of coagulation and fibrinolysis have been identified that lead to thrombosis or bleeding. These include von Willebrand disease (vWD), hemophilia, coagulation factor deficiencies, acquired/congenital inhibitors of coagulation, and antiphospolipid syndrome. 

As this patient has decompensated cirrhosis with ascites and a documented history of repeated episodes of SBP, an lUD cannot be considered as this method is contraindicated in patients prone to infection and experiencing disorders of coagulation.
