The approach is not a satisfactory method of identifying mistaken intentions or diagnostic errors. 

Unless specifically contraindicated the pupils of both eyes should be dilated rather than dilating only one eye for initial examinations. Failure to dilate the pupil of the contralateral eye can cause diagnostic errors because lesions considered to be normal variants frequently occur bilaterally. In addition, the contralateral eye can serve as a control, or normal eye, for that individual. Once a lesion has been documented, on subsequent visits the affected eye can be dilated alone, with the unaffected eye being dilated on a routine periodic basis. However, dilating one eye only could be determined by patient preference due to the Pulfrich effect. 

Penicillamine can cause an artificially abnormal alpha-1 antitrypsin protein pattern, which can result in diagnostic errors (404).

Anxiety is a subjective emotional and physical state experienced by all at some point. Anxiety becomes problematic when it cannot be controlled and begins to interfere with an individual s ability to work, sleep, or concentrate. More than one-third of all clients who seek mental health treatment present with some type of anxiety-related problem, yet due primarily to diagnostic error only one in four people with anxiety disorders receive adequate intervention (Hales, 1995). It is alarming that so many individuals who suffer from anxiety are incorrectly diagnosed or inadequately treated, because most anxiety disorders respond to intervention with short-term success rates as high as 70% (Roth Fonagy, 1996). 

Medical error is an umbrella term given to all errors that occur within the healthcare system, including mishandled surgery, diagnostic errors, equipment failures and medication errors. As medicines are the most common interventions in the healthcare system, medication errors are probably one of the most common types of medical error. Research suggests that approximately 7000 patients a year are killed by medication errors in the USA (Kohn et ai, 1999), and in British hospitals the incidence and consequences appear to be similar (Cowley et ai, 2001 Dean et ai, 2002).

Physician-related problems include lack of skill, diagnostic error, and the use of inappropriate force. Patient-related problems include abnormal bony structure, pathologic entities, intolerance of head motion leading to vertigo, and psychological intolerance of this type of manipulation.
