Cyproheptadine can decrease ACTH secretion in the Cushing s disease patient. Morning plasma cortisol concentrations, as well as 24-hour urinary cortisol (free) concentrations should be monitored. Side effects are minor and include sedation and hyperphagia. Cyproheptadine should be reserved for nonsurgical candidates who fail more conventional therapy. Because the response rate is no more than 30%, patients should be followed closely for relapses. 

Cyproheptadine (Periactin) Serotonin and cholinergic antagonist which may inhibit secretion of ACTH from pituitary microadenoma cells. Experimental agent for treatment of ACTH hypersecretion and Cushing s disease. 

Cyproheptadine and reserpine (10" -10 M of each) suppressed immunoreactive ACTH and P-endorphin secretion from adenomatous and non-adenomatous tissues of patients with Cushing s disease (SuDA et al. 1983). Inhibition of depolarisation-dependent Ca entry into cells as suggested by Do-NATSCH et al. (1980) in insuUn secretion or a nonse-rotonergic mechanism may be involved.
