Compressed air hoses

Prohibition on playing around with compressed air hoses, e.g. aiming directly at any individual. 

According to the OSHA investigation report, the reactor was undergoing a regular maintenance procedure for the removal of a solidified polyethylene blockage on three of the six reactor settling legs. The procedure called for the isolation of the 8-inch valve and the removal of the compressed air hoses used to operate the valve. [7]... 

After the explosion, investigators confirmed that the 8-inch DEMCO block valve was open at the time of the accident. Furthermore, tests showed that the compressed air hoses were improperly connected in a reversed position. This unauthorized step during maintenance...

Using a compressed-air hose as a toy is particularly hazardous, especially if the air-stream is brought in too close or in direct contact with any portion of a worker s body or clothing. If this occurs and there is a break in the skin, air may be forced into the bloodstream, often with fatal results. 

Compressed air hoses used on concrete pumping system shall be provided with positive fail-safe joint connectors to prevent separation of sections when pressurized.
