Me3SiCl 14 (40 mmol) is added with stirring to a solution of 20 mmol benTyl benzoate and 40 mmol Nal in 20 mb abs. acetonitrile. The reaction mixture is then heated under reflux for 4 h, cooled to room temperature, and 50 mb H2O is added to saponify the trimethylsilyl benzoate. The reaction mixture is then taken up in 2 X 50 mb ether, which is washed successively with H2O and aqueous thiosulfate solution to remove inorganic salts and iodine. The benzoic acid is then extracted with 2 X 15 mb aqueous 15% NaHC03 solution, leaving benzyl iodide and traces of unreacted benzyl benzoate in the ether layer. On acidification of the aqueous NaHC03 extract 80% benzoic acid is recovered [1] (Scheme 6.17). 

USP received eight reports Ifom pharmacists expressed concern about the labeling on the Bentyl (dieyclomine hydrochloride antispasmodic) 2-mL ampule. Practitioners reported that the label indicates only the drug concentration, 10 mg/mL, not the total volume, and some regarded the label information as incomplete. In one report, a 20-mg dose was ordered, but two ampules were administered (a total of 4 mL instead of 2 mL), leading to an overdose. This happened because the practitioner mistook 10 mg as the total eontents of one vial. 

Dicyclomine inhibits muscarinic action of acethylcholine on postganglionic parasympathetic effector regions. It is used in combination with other drugs for treating stomach ulcers, colic in children, and for treating irritable bowel syndrome. Synonyms of this drug are anaspaz, bentyl, dibent, and formulex.

Dicyclomine (Bentyl), oxybutynin (Ditropan), and tolterodine (Detrol) are nonselective smooth muscle re-laxants that produce relatively little antagonism of muscarinic receptors at therapeutic concentrations. The mechanism of relaxation is not known. Finally, some other classes of drugs can act in part as muscarinic antagonists. For example, the antipsychotics and antidepressants produce antimuscarinic side effects (e.g., dry mouth). 

Nonselective antimuscarinic drugs have been employed in the therapy of peptic ulcers (see Chapter 40) because they can reduce gastric acid secretion they also have been used as adjunctive therapy in the treatment of irritable bowel syndrome. Antimuscarinic drugs can decrease the pain associated with postprandial spasm of intestinal smooth muscle by blocking contractile responses to ACh. Some of the agents used for this disorder have only antimuscarinic activity (e.g., propantheline), while other drugs have additional properties that contribute to their antispasmodic action. Dicyclomine (Bentyl) and oxybutynin (Ditropan) at therapeutic concentrations primarily have a direct smooth muscle relaxant effect with little antimuscarinic action.
