Despite the significant benefit that tamoxifen has bestowed on breast cancer patients, the third-generation aromatase inhibitors are rapidly replacing tamoxifen as the first-Une treatment for breast cancers. In this chapter, focus will be given to three representative small-molecule aromatase inhibitors for breast cancer exemestane (1, Aromasin ), anastrozole (2, Arimidex ), and letrozole (3, Femara ). [Pg.33]

Exemestane, Excmestanc, Aromasin. is an irreversible aromatasc inhibitor related structurally to androslenedi-one. It is. supplied in 2S-mg tablets that al.so contain mannitol. mclhylpaniben, and polyvinyl alcuhol. Tlic u.sual dose is 25 mg once daily after a meal. The drug is rapidly absorbed. bound 90% to plasma proteins, and its blu levels decline exponentially with a mean terminal half-life of about 24 hours. Extensive metabolism includes oxidations of the 6-mclhylcnc group and reduction of the 17-kclo group. [Pg.438]

Exemestane (Aromasin) is a more potent, orally administered analog natural substrate androstenedione that lowers estrogen levels more effectively than formestane. In contrast to the reversible competitive inhibitors anastrozole and letrozole, exemestane irreversibly inactivates the enzyme, and, therefore is referred to as a suicide substrate. Doses of 25 mg per day inhibit aromatase activity by 98%, and lower plasma estrone and estradiol levels by about 90%. It... [Pg.260]
