Drugs that may affect gabapentin include antacids, cimetidine, hydrocodone, and morphine. Drugs that may be affected by gabapentin include oral contraceptives and hydrocodone.

Gabapentin. This agent is used as an adjunctive agent and is relatively safe from side effects in nonpregnant adult patients. There was noted a small decrease in gabapentin excretion with concurrent cimetidine administration, which was not clinically significant. It is recommended that gabapentin be taken at least 2 h after antacid administration. 

Drug interactions are infrequent with gabapentin. It does not induce hepatic metabolizing enzymes, nor do other AEDs affect its metabolism and elimination. Antacids may decrease absorption. Gabapentin dosage may need to be decreased in patients with renal disease or in the elderly. 

An aluminium/magnesium hydroxide antacid Maalox TO) reduced the bioavailability of gabapentin 400 mg by about 20% when given either at the same time or 2 hours after gabapentin. When the antacid was given 2 hours before the gabapentin, the bioavailability was reduced by about 10%. These small changes are unlikely to be of clinical importance. How-...
