Healthcare is an extremely diverse enterprise and the causes of harm, and the associated solutions, will differ according to the process under consideration. Some factors, such as leadership, culture and attitudes to safety, are generic and important in all environments. However, the kinds of specific solutions required to ensure high reliability in, for instance, blood transfusion services, will obviously differ horn those aimed at reducing inpatient suicides.
