Warfarin Acarbose

Acarbose may increase the availability of warfarin (81). However, neither miglitol (82) nor voglibose (83) has any effect.

Rosiglitazone has been studied in combination with acarbose, digoxin, metformin, nifedipine, ranitidine, warfarin, and oral contraceptives [67,72] There have been no clinically significant effects on the pharmacokinetics of any of these agents in combination with rosiglitazone. 

Acarbose, miglitol and voglibose did not appear to alter the pharmacokinetics or effects of warfarin. However, there are a few cases of reduced or increased INRs in patients given warfarin and acarbose. 

The picture presented by these cases is that usually no interaetion oeeurs, but in isolated cases some changes in warfarin requirements oeeur. Bear this potential interaction in mind if anticoagulant control alters in a patient taking acarbose.
