A--Strophanthin

Stiripentol [inn, usan] (BCX 2600) is a benzodioxolyl derivative, an ANTICONVULSANT and anxiolytic. K-stophanthin-a strophanthin-K. stophanthoside-K strophanthin-K. 

Obt. by add hydrol. of A-Strophanthin-P isol. from seeds of Strophanthus kombe. Sugar from the glycosides K-Strophan-thoside and Echujin.

The pharmacokinetics of cardiac glycosides (A) are dictated by their polarity, i.e the number of hydroxyl groups. Membrane penetrability is virtually nil in ouabain, high in digoxin, and very high in digitoxin. Ouabain (g-strophanthin) does not penetrate into cells, be they intestinal epithelium, renal tubular, or hepatic cells. At best, it is suitable for acute intravenous induction of glycoside therapy. 

A mixture of cardiac glycosides isolated from Strophantus kombe mainly contains K-strophanthin- 8 and K-strophanthozide. 

Strophanthin Strophanthin is made up of a mixture of glycosides, mainly K-strophan-thin-p, 3p,5,14-trihydroxy-10-oxo-5P-card-20(22)-enolide-3-D-cymaro-p-D-glycoside (17.1.3), and K-strophanthoside, 3p,5,14-trihydroxy-10-oxo-5p-card-20(22)-enolide-3-D-cymaro-p-D-gluco-a-D-glycozide (17.1.4). Diatrizoate and strophanthin K may have a synergistic toxic effect (312). 

Note 9 The authors mentioned that sparfloxacin, grepafloxacin, moxifloxacin, and ciprofloxacin (3, 10, 30, and 100 pg/ml) prolonged AP duration in a concentration-dependent manner. The assay is useful for certain compounds, which specifically decrease the conduction velocity of purkinje fiber (K-strophanthin in canine purkinje fiber).
