US incidents in the twenty-first century

Received lots of help for Chapter 4 Major US Incidents in the twenty-first century. Dr Vic Edwards was kind enough to review this chapter as it was being developed and his input was appreciated and useful. 

This chapter provided a review of four major US incidents of the twenty-first century. These incidents were identified by OSHA as examples of incidents that should not ever be repeated. If there is additional interest in details, the first three incidents have been well documented and reports are available within CSB reports and videos. These resources are available on the web.
