Fig. 634 The lead to be removed is encircled by the deflecting wire. The Amplatz snare, creating a continuous loop, grabs the deflecting wire. Closing the Amplatz snare, locking the loop with a clamp, and retracting it into the Byrd Femoral Work Station closes the loop. The Byrd Femoral Work Station is advanced over the lead all the way to the lead tip, freeing it from the endocardial attachment. (From Belott PH. Endocardial lead extraction A videotape arul manual. There are several techniques available for mechanical thrombolysis. The most common is probing the thrombus with a microguidewire. This technique appears to be useful in facilitating chemical thrombolysis. Alternatively, a snare (e.g., Amplatz Goose-Neck Microsnare, Microvena, White Bear Lake, MN) can be used for multiple passes through the occlusion to disrupt the thrombus. " A snare can also be used for clot retrieval, mostly in simations in which the clot has a firm consistency or contains solid material. ... [Pg.80]

Recanalization of tight occlusions of the left innominate vein, which has a long mediastinal course, as may be caused by mediastinal tumor or fibrosis, can require a combined femoral and axillary or a bilateral axillary approach (Fig. 5.6). When the guidewire has crossed the occlusion, it is grasped with a snare, a basket, or a flexible forceps on the other side, and retrieved through the haemostatic valve sheath at the second percutaneous entry point. This makes it possible to insert a PTA catheter or any other stent delivery catheter over a stiff Amplatz-type guidewire, which is controlled and straightened at both ends. [Pg.123]
