Selegiline dosage

When used as a monotherapy in the early stage of Parkinson s disease, selegiline provides a mild antiparkinsonian effect. When given concomitantly with L-dopa, it potentiates the effects of L-dopa and allows a reduction in daily L-dopa dosage. In patients with fluctuating responses, selegiline decreases their severity, particularly the wearing off, but less so the off-on phenomenon. 

From the evidence available to date, the reversible, short-acting MAO inhibitor moclobemide, which is available in several countries (but not the USA), appears to be relatively free of this interaction. (The selective MAO-B inhibitor selegiline loses selectivity at antidepressant dosage. Because its action is on the enzyme that metabolizes dopamine, it is most useful in the treatment of Parkinson s disease [Chapter 28 Pharmacologic Management of Parkinsonism Other Movement Disorders].)... 

Dosage of carbidopa-levodopa can at times be reduced by f 0-30% after 2-3 days of adminisfering selegiline 5-f 0 mg/day in Parkinson s disease...
