Reports are made online by completing the appropriate online report form. The form will be submitted to the RIDDOR database and a copy sent back to the employer for his records. [Pg.16]

One port operator s safety department examined the accident statistics for the past six years for two major commercial ports. In that time, the two ports had had 4 reportable construction accidents and since then have spent around 100 million in capital expenditure. Two of these accidents occurred in the marine environment but were not the result of a marine-related risk, i.e. they could just as easily have occurred on land. However, the port s accident data may mask the true picture, as they would not record the contractor s accident data, as contractors are separately responsible for submitting the RIDDOR forms to HSE. [Pg.45]

All accidents on or around the site are reported and investigated by management. Any changes found necessary are quickly implemented. All accidents, reportable under RIDDOR 1995, are reported to the HSE on form F2508. [Pg.79]

This database was developed in the United Kingdom, and it contains supplementary information dating back to October 1992, on all types of offshore hydrocarbons releases reported to the HSE OSD under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR) 1995 and earlier offshore legislation. The information is submitted voluntarily on the form called OIR/12 form and is also recorded in a separate and specifically designed database maintained by the HSE OSD. [Pg.135]
