Lysodren

Mitotane, a derivative of the insecticide DDT, quickly lowers the level of corticosteroids, and is metabolized in the blood and urine and used on non-operable metastatic prostate carcinomas. Synonyms of this drug are lysodren and others. 

The observation that mitotane (Lysodren) could produce adrenocortical necrosis in animals led to its use in the palliation of inoperable adrenocortical adenocarcinomas. A reduction in both tumor size and adrenocortical hormone secretion can be achieved in about half of the patients taking the drug. Because normal adrenocortical cells also are affected, endogenous glucocorticoid production should be monitored and replacement therapy administered when appropriate. 

Mitotane (Lysodren) produces selective atrophy of the zona fasciculata and zona reticularis, which results in a decrease in the secretion of 17-hydroxy corticosteroids. Direct inhibition of cholesterol side-chain cleavage and 11 (3/18-hydroxylase activities has also been demonstrated. Mitotane is capable of inducing remission of Cushing s disease, but only after several weeks of therapy and at the price of severe gastrointestinal distress. Mitotane. Although the exact mechanism of this drug is unknown, mitotane (Lysodren) selectively inhibits adrenocortical function. This agent is used exclusively to treat carcinoma of the adrenal cortex.

Mitotane (Lysodren) is administered in initial daily oral doses of 2 to 6 g, usually given in three or four divided portions, but the maximal tolerated dose may vary from 2 to 16 g per day. Treatment should be continued for at least 3 months if beneficial effects are observed, therapy should be maintained indefinitely. Spironolactone should not be administered concomitantly because it interferes with the adrenal suppression produced by mitotane.

Treatment with mitotane is indicated for the palliation of inoperable adrenocortical carcinoma, producing symptomatic benefit in 30—50% of such patients. Although the administration of mitotane produces anorexia and nausea in 80% of patients, somnolence and lethargy in 34%, and dermatitis in 15—20%, these effects do not contraindicate the use of the drug at lower doses. Since this drug damages the adrenal cortex, administration of corticosteroids is indicated, particularly in patients with evidence of adrenal insufficiency, shock, or severe trauma.
