Disastrous incidents continue to occur even though dedicated professionals have worked diligently within their organizations for decades and with the OSHA PSM standard since 1992. Most US chemical and petroleum industries have been working for about two decades to comply with all the elements of the OSHA PSM standard. Many international and some non-US facilities have adopted the OSHA PSM standard or created other regulations to reduce the risk of incidents.
