How Do You Prevent Such an Incident

The newspaper piece said that the truck was properly manifested, and that four separate individuals should have checked each chemical delivery prior to it being unloaded. 

Apparently, the procedure was either not followed, or there was a miscommunication. Maybe it had something to do with the fact that this occurred in a unit that was started up about three months before with 38 new employees. [9]
