Good practice mitigation measures

Records Hazards mitigation measures (by reference to authoritative good practice or the Hazard Log) confirmation that these mitigation measiues are being put into practice. 

We may well carefully document our hazards and risks, and detail out the mitigation and control measures required for safety, but do not seem to be as good at putting this into practice within the complex, changeable site environment. 

Good measurement practices to mitigate artifacts, noise, and interference.
