Imaging with PET or contrast enhanced cardiovascular magnetic resonance can identify areas of LV scar. The placement of the LV lead at areas of LV scarring will show no response to CRT and can lead to worsening of heart failure due to unopposed RV pacing (75) or worsening of ventricular tachycardia (76). Bleeker et al. showed that patients with transmural scar in the posterolateral LV (as assessed by MRI) did not show clinical or echocardiographic response with CRT (77).
