Annual inspection

The results of corrosion surveys are received by the Surveyor in an agreed format using Forms TMl - TM8, preferably with the LR software freely issued to the approved firms. The report will highlight any areas where corrosion has gone beyond the permitted maximum diminution, which may be as low as 15% of the nominal thickness for a single item, and will also identify areas of "substantial corrosion T.e., any in excess of 75% of the permitted maximum. Corrosion in excess of that permitted means that the item has to be replaced. Any areas of corrosion deemed to be "substantial" are recorded for annual inspection. The Surveyor will review the completed report for verification and confirmation of the completeness of the thickness survey. 

In cases where a lot of proplnt has successfully passed the one-year exposure to methyl violet paper, only a 3% inspection of the exposed test strips need to be made thereafter until such inspection indicates progressive deterioration of the proplnt or other nonstandard condition, at which time 100% inspection and test of the lot under suspicion will be resumed It is considered desirable that succeeding annual inspections include at least 1/3 of the containers included in the original 3% inspection as outlined above. In this manner, such containers may be considered as a basic comparative control with reference to the balance of the lot under test... 

The inspection is undertaken primarily to ensure the continuous safe operation of the equipment and to direct repairs or changes in operation if questions of safety or adequacy for service arise. To be of significant value, an inspection must be carried out thoroughly and the results and conclusions compared with those of previous inspections. Local or state authorities may demand annual inspection and provide for penalties if a violation of the appropriate industrial pressure vessel or administrative code is found. 

Recent developments in organic inspection and certification focus on the development of a risk-oriented approach, in addition to the annual inspection visit. When inspecting the operators, the amount of time spent must be flexible and should be more orientated to goals and risks. Sanctions must be effective and strongly discourage violations. 

Fire pumps should have a weekly and annual inspection and test. Specific requirements for fire pump inspection, testing, and maintenance are contained in NFPA 25. 

Annual inspection, testing, and maintenance consist primarily of a full flow test of the pump. The flow test should be conducted at three points no flow (churn), 100% of rated flow capacity and 150% of rated flow capacity. The results of the flow test should be compared to previous flow tests and the design curve. Increasing the engine speed beyond the rated speed of the pump at rated conditions is not an acceptable method for meeting the rated pump performance. If the pump curve is more than 10% below the last flow test, then this indicates a potential problem with the pump and additional maintenance may be required. 

An annual inspection should be made of foam concentrates and their tanks or storage containers for evidence of excessive lumps or deterioration. Samples of concentrates should be sent to the manufacturer or qualified laboratory for quality condition testing. 

The annual inspection consists of a thorough inspection and test for proper operation of the entire system, including the detection system. The enclosure should be inspected for new penetrations that could affect agent leakage and system performance. The enclosure inspection is not required if a documented... 

While leak detection is not emphasized for aboveground storage tanks (ASTs), daily inspections, monthly inspections, annual inspections, and five-year inspections are legally required by the State of New York for AST owners or operators [14]. 

By making expectations crystal clear and creating a strong likelihood that those in violation will be punished, Congress and OSHA may have offset the fact that enforcement is so episodic and, therefore, may have secured compliance similar to that that would have been achieved if annual inspections were feasible. However, doing this implies that the particular firms that happen to be inspected may find themselves subject to far more rigorous enforcement of the rules than reasonable protection of worker health and safety requires. 

Since BkPdr deteriorates with the absorption of moisture and the containers in which it is stored are subject to rust, a thorough inspection of these articles must be made at least once a year. For this, one or several containers from each lot will be opened at the time, of annual inspection and if there is any doubt about serviceability of the pdr, it must be removed to the lab for physical and chemical testing. At the same time, the containers must be inspected for holes, weak spots, etc. Damaged containers should not be repaired but removed and the contents transferred to new containers. All of these operations may be called surveillance... 

Such a survey becomes a combination of an annual inspection and a troubleshooting exercise. The fact is that operating practices change with time the production process may be adjusted or new sections of cooling systems are added to the main plant, or pumps, heat exchangers, and other items of equipment are periodically replaced. The personnel also change. 

Regular inspections should be carried out on a daily, weekly, or annual basis, as appropriate. Annual inspections should include all the areas previously discussed in Chapter 8. 

An inspection program for field trials was developed that provides reasonable assurance that the studies are being conducted according to the protocol and the GLP regulations. It was deemed impractical to inspect each trial so the procedures used by the field scientists are inspected and it is assumed that all of the trials done by that trialist are conducted similarly. At least annual inspections are made at each site except for complex studies where multiple inspections are usually made. 

Conrad s Centralia plant has no pollution control equipment except for the outside flare for the excess gas. No continuous emissions monitoring systems are used. No local regulations apply to the facility, although an annual inspection is conducted on site by regulatory agencies. 

The certifying body carries out an annual inspection of premises, systems and production records to ensure that all of these procedures are followed. 

To summarize this section, PT programs are far from ideal monitors of laboratory performance. In a study of PT survey problems at the Mayo Clinic, more than one half of the errors on surveys were directly related to deficiencies in the surveys (such as invalid specimens and inappropriate evaluation criteria), and only 28% could be linked to specific analytical problems. A novel PT program piloted by the New York State Department of Health involves obtaining aliquots of patient specimens that are routinely processed by the laboratory at the time of its annual inspection. These specimens are reanalyzed in the state s laboratories using reference methods, and the results are compared with those obtained during inspection. This prevents problems such as the presence of unnatural specimens and the clerical errors of foreign forms. Other specimen stability problems and methodological interferences probably continue to exist. 

The number of annual inspections has been growing steadily, and in 2004, reached a total of 242 for US clinical investigators and 82 for foreign clinical investigators (2004 Report to Nation). The top five deficiencies, led by protocol violations, appear in Figure 11.9. 

The audit system is overseen by the Responsible Care Review Committee. Two levels of audit are described. Internal audits are reviewed annually and top executives carry out annual inspections of all plants and laboratories. 

Financial obstacles during the conversion period were relevant for the organic farmers. Relatively high costs of initial certification and the annual inspection, which are dependent on the certification body and also partly on the size of the farm, are also seen as problematic. These costs can often not be recouped, especially during the conversion period, as farmers frequently lack possibilities... 

It is essential for steady and safe operation of a sodium cooled fast reactor to limit the coolant and cover gas impurities to prevent corrosion of reactor component materials and to reduce radiation dose by corrosion products (CPs). Therefore, impurity concentrations of both coolant sodium and cover gas argon were measured during the duty cycle operation and annual inspection period. The sodium impurity data include oxygen, hydrogen, nitrogen, chloride, tritium, metal elements and radioactive ° Ag, Na, Xs. The cover gas impurity data include O2, N2, CO, CO2, H2, CH4, He, H and radioactive xenon and krypton isotopes. 

The CP deposits on the inner surfaces of the primary main piping have been measured at every annual inspection period since the end of MK-I operation in July 1982. These measurements are made at 14 locations, shown in Fig. 18, using a Ge solid-state detector system. The detector system was calibrated with a piping mock-up using two planer type standard gamma-ray sources, " Mn and °Co, so that the absolute amounts of CP deposits could be obtained from the measured gamma-ray spectra. 

Later at the annual inspection of the blower, it was found that there was a loss of lubrication oil in the bearing box and the vibration level decreased after refilling the oil. 

A sodium leak accident occurred in the MONJU secondary cooling loop in December 1995. At that time, the 11 annual inspection was being performed at JOYO and an inspection of sodium piping and components was immediately carried out to confirm their integrity and to verify that there were no sodium leaks. After the MONJU accident, an investigation was completed by the Safety Authority and findings on the cause of the sodium leaks and ways to mitigate their effects were published. 

BOEMRE inspectors and investigators are based in 7 district offices (5 in the COM, r in California, r in Alaska) and fly offshore regularly. OCS Eands Act mandates annual inspections. Inspectors have authority to issue Incidents of non-compliance which may be a warning or a facility shut-in. Civil penalties may be issued when violations pose actual harm or threat of harm to personnel or the environment. Industry self-inspections and records are required by regulation. Drills (spill response, BOPE, H2S, evacuation, etc.) are required and may be initiated by BOEMRE without notice. Third-party reviews (design, fabrication and installation) are required for deepwater or novel structures. 

These plans must address worst-case discharge and response capabilities including equipment and response time. BOEMRE conducts annual inspections of equipment and conducts drills to test industry response. BOEMRE shares responsibility with the U.S. Coast Cuard (USCG) in incident response. The USCG acts as the federal on-scene coordinator and leads the operational spill response, while BOEMRE focus is on source control. 

The annual inspection requirement can be met in one of the following three ways ... 

If a mandatory state inspection program listed in No. 1 does not apply, a vehicle could meet the federal annual inspection if it passed a Commercial Vehicle Safety Alliance (CVSA) roadside inspection and has been issued an inspection decal during the preceding 12 months. The motor carrier must retain a copy of the inspection report and ensure the inspection meets federal requirements.

Type C service includes all items of a Type A and Type B service and any needed more extensive service such as an alignment or complete brake overhaul. Some carriers will also perform the required periodic (annual) inspection as part of the Type C service.
