Digoxin Allopurinol

Drugs that may affect ACEIs may include antacids, capsaicin, indomethacin, phenothiazines, probenecid, and rifampin. Drugs that may be affected by ACEIs include allopurinol, digoxin, lithium, potassium preparations/potassium-sparing diuretics, and tetracycline. 

No clinically important pharmacokinetic interactions appear to occur when famciclovir is given with allopurinol, digoxin or theophylline. 

Drugs that may be affected by antacids include allopurinol, amphetamines, benzodiazepines, captopril, chloroquine, corticosteroids, dicumarol, diflunisal, digoxin, ethambutol, flecainide, fluoroquinolones, histamine H2 antagonists,... 

A 69-year-old man with hypertension and heart failure took losartan 25 mg/day, increasing to 50 mg/day after 2 weeks. He also took spironolactone 50 mg/day, furo-semide 40 mg/day, digoxin 0.25 mg/day, acenocou-marol, and allopurinol. Two weeks later he developed acute renal insufficiency with a plasma creatinine concentration of 725 pmol/l (previously 115 pmol/l). Within 24 hours after losartan withdrawal (it was not stated whether spironolactone was also stopped) and hemodialysis, he recovered renal function (plasma creatinine 124 mg/1). He was later found to have bilateral renal artery stenosis, which is a contraindication to angiotensin II receptor antagonists.

A woman taking digoxin, furosemide, spironolactone, allopurinol, and warfarin (prothrombin ratio 2.8) developed haematemesis within 4 days of starting to take azapropazone 300 mg four times a day. Her prothrombin ratio was found to have risen to 15.7. Subsequent gastroscopic examination revealed a benign ulcer, the presumed site of the bleeding. ... 

Allopurinol does not appear to affect serum digoxin levels. 

No significant changes in the serum digoxin levels of 5 healthy subjects occurred over a 7-day period while they were taking allopurinol 300 mg daily. No additional precautions would appear to be necessary on concurrent use.
