Blind eyes

Regiilar checks and audits should be made to confirm that they are being carried out correctly. They will corrode more rapidly than the steelwork, once those in charge lose interest or turn a blind eye.  [c.2269]

You may believe that the accidents could not happen at your plant because you have systems to prevent them. Many of the accidents I describe occurred on plants that had such systems, but the systems were not always followed. The accidents happened because of various management failures failure to convince people that they should follow the systems, failure to detect previous violations (by audits, spot checks, or just keeping an open eye), or deliberately turning a blind eye to avoid conflict or to get a job done quickly. The first step down the road to many a serious accident occurred when someone turned a blind eye to a missing blind (see Chapter 1).  [c.426]

Deluxe Phosphors. Because the two complementary emission bands from the calcium halophosphate phosphor do not fill the visible region of the spectmm and in particular are deficient in the red region of the spectmm, colors are distorted under these lamps compared to their appearance under blackbody radiator sources or sunlight. This distortion is measured by color rendition indexes for different colors and by the average color rendition index Ra. To improve color rendition, blends of other broad-band emitting phosphors are used in some fluorescent lamps. In particular, strontium or strontium calcium barium orthophosphate activated with tin is commonly used to provide a broad-band red emission peaking near 620—630 nm. This phosphor is blended with strontium halophosphates. The strontium halophosphates are completely analogous to the calcium halophosphates but with the Mn band shifted toward 560 nm and the Sb " band shifted toward the green. Using a blend of the broad-band green strontium halophosphate phosphors together with the broad red emission of the orthophosphate there is continuous emission of radiation across the visible region of the spectmm and good color rendering Delux-type lamps can be made. However, because the broad-band emissions extend outside of the range of the eye sensitivity function and because of rather low quantum efficiencies of these phosphors, the lamps only have about two-thirds of the light output of standard halophosphate lamps.  [c.288]

Reliability prediction is undoubtedly an important aspect of the product design process, not only to quantify the design in terms of reliability, but also to determine the critical design parameters that go to produce a reliable product. To this end, it is necessary to have a mathematical, quantitative measure of reliability defined by probability (Leitch, 1995). It is, however, a controversial aspect of reliability engineering in terms of accuracy and validity because it relies on detailed knowledge about sometimes unknown design parameters (Burns, 1994 Carter, 1986 O Connor, 1995). In addition, the practical engineering of reliability in the product is still seen as a science to many designers and evaluating concept designs for reliability is especially difficult for inexperienced staff (Broadbent, 1993). A fundamental reason for this is the supporting use of statistics and probability theory. Designers and engineers have consistently turned a blind eye to the advantages of using these methods however, they can be trained to use them without being rigorously schooled in their mathematical foundation (Morrison, 1997). Reliability prediction will remain a controversial technique until the statistical methods for quantifying design parameters becomes embedded in everyday engineering practice. Using statistically based design techniques will not solve the whole problem, although it will be much closer to the desired end result (Carter, 1997).  [c.30]

Investigation showed that the foreman and manager knew that goggles were not always worn. But they turned a blind eye to avoid dispute and to avoid delaying the job. The workers knew this and said to themselves, Wearing goggles cannot be important. The foreman and manager were therefore responsible for the inevitable injury.  [c.26]

The recommendations described in this Chapter go further than some companies consider necessary. For example, companies may put Do Not Operate notices on valves instead of locks, or to save time, they may turn a blind eye to occasional shortcuts. Nevertheless, bitter experience has convinced me that the reconamendations are necessary.  [c.44]

Stick Molding. Various types of Hpsticks and eye-shadow sticks are stick molded. A wax-containing Hpid mass is milled with the pigment at elevated (about 75°C) temperatures until it is uniform. The Hpid mass, at a temperature about 10°C above its melting point, is then poured into metallic molds. Deaeration is essential to prevent unsightly depressions on the molded sticks. To avoid sudden congealing, the molds are customarily heated to a temperature above room temperature before filling after filling, they are chilled to temperatures weU below room temperature. After unmolding, the sticks may be inserted into various types of containers (swivel, metal, or plastic). In order to formulate acceptable molded sticks, slowly developing surface anomaHes or defects must be avoided. Foremost are the excrescence of soHd fatty substances (also called bloom) and the exudation of Hquid substances (also called sweating). Both of these defects are attributed to polymorphic transformation. The selection of the proper blend of Hpids to create an acceptable makeup stick is complex. Some of the Hpids used in such products and their primary characteristics are Hsted in Table 10. Other types of sticks, for example, eyebrow pencils and Hp liners, are molded similarly but may be inserted into wooden pencil stock, trimmed, and appropriately finished.  [c.294]
